前台翻了翻,抬头问了一句:“Are you the legal guardian?”

The minor named below lives in my home and I am 18 years of age or older.”
加州法院自助中心的说明是:“Parents do not need to sign the form, but they can cancel it at any time.”

第 6552 条的法定表格里,“A seal or signature from a court is not required. This form is not required to be notarized.”
那它为什么叫 affidavit(宣誓书)?签名上方那一行给出了答案:“I declare under penalty of perjury under the laws of the State of California that the foregoing is true and correct.”——依伪证罪处罚之声明。
The Caregiver’s Authorization Affidavit does not expire.”(不会过期);而法院自助中心的网页上仍写着”You must sign a new Caregiver’s Affidavit each year.”(每年要重签一份)。
